IMMEDIATE OPENINGS throughout the Capital District (Troy, Albany, Schenectady, Latham, Watervliet, East Greenbush) Full and Part time hours needed 12 hour shifts available
Responsibilities:
Assist with personal care activities, including bathing, feeding, dressing and mobility exercises.
Assist with household tasks.
Performs other appropriate duties as assigned by the supervising nurse on client Plan of Care.
Submits and maintains all necessary documentation electronically including arrival to shift, tasks performed, services, notes, and departure from shift.
Be aware of potential problems in the home environment that can affect a client's health status.
Maintain a high level of confidentiality when working with clients and families.
Communicate any changes in client's condition or incidents to the RN supervisor.
Reliable transportation required - personal vehicle, taxi, or ride sharing - you must be able to get to work on your own.
