Eddy SeniorCare is the Capital Region's only Program of All-Inclusive Care for the Elderly (PACE).
PACE is a special model of care proven effective at helping older adults with long-term health care needs live safely at home.
Eddy SeniorCare is currently looking for a Full-Time HHA to work out of our brand new PACE center in Latham, New York.
This new program is located on the beautiful 83 acre, hillside campus of the Sisters of St.
Joseph Eddy Senior Care supports seniors at home by bringing together under one roof a wide array of medical care, home health care, therapy, and other support services.
In the PACE model of care, each participant's health and support services are managed by a care team that is based at our agency.
The team includes doctors, nurses, therapists, home health aides, day center staff, transportation drivers, nutritionists, social workers, and others.
The PACE one-roof team model of care improves care coordination and responsiveness.
Responsibilities:
The Certified Home Health Aide is a paraprofessional health care worker prepared to function in a supportive relationship to professional nursing and other health care disciplines.
The Home Health Aide provides, under appropriate professional direction and supervision, health care services and other supportive services in the home as defined in the patient's Plan of Care.
What you will need:
High School diploma or GED Completion of Home Health Aide Training Program, Home Health Aide Equivalency Program, C N A Transition Program, or Personal Care Aide Upgrade Program approved by the New York State Department of Health New York State Drivers License One or more years experience is preferred, as either a Home Health Aide in a Certified or Licensed Home Health Agency or as a Certified Nursing Assistant in a hospital or nursing home.
